STUDENT INSURANCE

The Caldwell Parish School Board shall make available student accident insurance for purchase for students attending Caldwell Parish public schools.  Application forms shall be sent home with students during the first week of school.  The schools shall not be liable for any premium payment. 

RELEASE FORM FOR ATHLETES

All students engaging in interscholastic activities shall be required to fill out a form which releases the school and Board from any liability for accidents or injury to student athletes. This release form must be signed by parents before the student shall be permitted to participate in athletics.
